Job summary

Health Watch Provo in Utah is seeking a compassionate Registered Nurse to provide on-call coverage one week per month for our Home Health and Hospice patients. The On-Call RN serves as a critical clinical resource, delivering after-hours assessment, triage, and care coordination.

The ideal candidate will have strong clinical assessment, effective communication, and the ability to work independently while collaborating with physicians and the interdisciplinary team.

Qualifications

• Active Utah RN license (unrestricted).
• Home Health or Hospice experience preferred.
• Strong assessment, critical thinking and clinical decision-making.
• Excellent communication and interpersonal skills.
• Ability to work independently and manage urgent situations.
• Reliable transportation with valid driver's license and auto insurance.

Responsibilities

• Provide on-call nursing coverage for one designated week each month, including evenings, weekends, and holidays as scheduled.
• Respond to patient, caregiver, and staff calls regarding changes in condition and care needs.
• Perform telephone triage and determine appropriate interventions.
• Conduct skilled nursing visits when clinically indicated.
• Coordinate care with physicians, case managers, therapists, hospice staff, and other health professionals.
• Assess, implement, and evaluate individualized patient care plans.
• Administer medications and monitor effectiveness and adverse reactions.
• Educate patients and caregivers on disease processes, medication management, safety, and resources.
• Document assessments, interventions, and changes in patient condition accurately and timely.
• Report significant findings to the provider and supervisor.
• Maintain compliance with agency policies and professional nursing standards.
