Qualifications

• One year or more of clinical RN experience.
• Current RN license for the states assigned.
• Current CPR certification.
• Valid driver's license and reliable transportation.

Responsibilities

• Performs patient assessments and collaborates with the care team to plan care.
• Makes referrals as indicated by patient needs and documents rationale.
• Promotes patient health through teaching and rehabilitative measures.
• Supervises LPNs and HHAs as needed.
• Completes documentation timely and communicates progress to the team.
• Plans staff education and participates in on-call duties.
