Job summary

Amedisys is seeking an After-Hours RN / On-Call RN (PRN) to perform patient assessments, coordinate with the care team, and provide on-call services in home health settings. RN license required; flexible scheduling and a focus on helping patients heal at home.

Qualifications

• 1+ year of clinical RN experience; RN license required for state assignment.
• Current CPR certification.
• Valid driver's license and reliable transportation.

Responsibilities

• Performs patient assessments and collaborates with the care team to develop and implement a plan of care.
• Makes referrals to other disciplines as indicated by the patient's identified needs.
• Promotes patient health through teaching and rehabilitative measures.
• Supervises LPNs and HHAs.
• Completes documentation timely and accurately.
• On-call responsibilities and communication of patient progress to the team.
• Plans staff education and participates in continuing education.
