Job summary

Alternate Solutions Health Network in Cincinnati, OH is seeking a Registered Nurse, On-Call to support a team of nurses and patients by responding to after-hours needs. You will provide one-on-one care in patients' homes and, when not required after hours, assist with chart reviews and patient calls from your home.

This role requires an active RN license in Ohio and at least one year of acute care experience; home care experience preferred.

Qualifications

• Minimum one-year experience as an RN in an acute care setting; home care preferred.
• Ability to communicate effectively with patients, families, physicians and co-workers.
• Ability to work autonomously and manage time efficiently.
• Detail-oriented, compassionate, and able to follow directions within scope of practice.

Responsibilities

• Observe and monitor patient conditions and perform assessments to develop care plans.
• Administer medications as prescribed and prioritize visits for high-risk patients.
• Oversee total care of patient with assistance from nurse aides and LPNs.
• Collaborate with interdisciplinary team to ensure optimal patient care.
• Complete chart reviews and respond to patient calls; document per protocols.
