Job summary

Alternate Solutions Health Network in Cincinnati, OH is seeking a Registered Nurse to provide comprehensive skilled care in patients' homes under physician orders, educate patients and families, and ensure safety and documentation compliance.

You will perform OASIS assessments, administer medications, coordinate with LPNs and aides, drive to patient locations, and participate in on-call requirements, while maintaining licensure and adhering to state regulations.

Qualifications

• Registered Nurse with active license in the state of employment.
• Minimum of one year of RN experience in an acute care setting.
• Home health experience is desired.
• Ability to communicate effectively with patients, families, physicians and co-workers.
• Valid driver's license and auto insurance in your name.

Responsibilities

• Provides skilled nursing care in a patient's home as ordered by the attending physician.
• Observes and monitors patient conditions and administers medication as prescribed.
• Performs OASIS assessments to develop and adjust an individualized plan of care.
• Promotes continuity of care with admissions, transfers and discharges.
• Counsels patient and family on disease processes and plan of care.
• Oversees total patient care provided by Aides and LPNs.
• Documents all treatment, assessments and patient education per agency protocol.
