Job summary

Heart to Heart Hospice is seeking a Registered Nurse On Call to provide compassionate home health visits, patient assessment, and crisis support. You will collaborate with physicians and the IDG to develop and revise care plans, ensuring timely documentation and responsive patient care.

This full-time role requires an active RN license, a valid drivers license, and 2 years of RN experience in healthcare settings.

Qualifications

• Graduate of an accredited Diploma, Associate or Baccalaureate School of Nursing.
• Current State Registered Nurse License.
• Current Driver's License with reliable transportation and ongoing auto liability insurance.
• Two years of RN experience preferred in a healthcare/clinical setting.

Responsibilities

• Provide home visits for patient assessment, treatment, and support, including pronouncement of patient death when needed.
• Develop and revise nursing care plans with the patient, physician, and IDG; plan comprehensive service for patients and families.
• Utilize excellent customer care skills with internal and external customers, including proper phone etiquette.
• Prepare clinical and progress notes showing progress toward goals.
• Inform physician and other personnel of changes in patient needs and outcomes; evaluate patient/family response to care.
• Timely submission of all documentation.
• Perform other duties as required.
• Ensure compliance with local, state and federal laws, Medicare regulations, and established personnel policies and procedures.
