Job summary

DCCCA, Inc. seeks a dedicated professional to support children and families during the referral period, delivering culturally competent services and applying evidence-based practice.

You will meet clients weekly, coordinate assessments, arrange medical appointments and referrals, and work with the case manager to develop recovery plans. The role requires on-call responsibilities and occasional weekends or holidays, drives to multiple DCCCA locations, and completion of pre-employment screenings.

Responsibilities

• Provides for the safety, stability and well-being of children and families during the referral period
• Delivers culturally competent services using a strengths-based approach to children and families consistent with the evidence-based practice models
• Meets DCF timelines to connect with the family and facilitate the initial family meeting
• Engages and works in partnership with the case manager and family to complete assessments
• Responds to calls and client emergencies as needed including being on-call and working some weekends and public holidays as necessary
• Meets with clients at least once a week to offer one-on-one support
• Develops recovery plans, coping methods and rehabilitation services with the client
• Facilitates treatment assessments and referrals and provides client support in arranging for medical appointments, counseling, support groups and other resources as needed
• Monitors the client’s behavior, schedule, medication and activities and reports any unusual behavior, conflicts or recovery deviations to the relevant professionals
• Provides transportation and resources and coordinates community support services as needed
• Job may require the employee to be able to regularly drive to various DCCCA locations and other business locations throughout the day or night as required
• Other duties as assigned
