BrightStar Care of Fort Wayne is seeking an on-call Home Health Aide (HHA) or Certified Nursing Assistant (CNA) to cover caregiver absences on a Set Team in Fort Wayne, IN.

The role requires one on-call day per week and one on-call weekend per month, with care provided to diverse clients. You will support scheduling and clinical teams while delivering dependable home health care.

Qualifications

• At least 1 year of home health experience as HHA or CNA.
• Current Indiana HHA or CNA credentials for the assignment.
• High school diploma or GED is required.
• Dependability and willingness to work with different clients.
• Comfort traveling to assigned homes in Fort Wayne, IN.
• Valid driver’s license, auto insurance, and reliable vehicle.
• Physical exam and negative TB test within the past 12 months.
• Ability to lift up to 50 pounds and assist with mobility.
• Strong English reading, writing, and verbal communication.
• HIPAA compliance; CPR preferred.

Responsibilities

• Review the care plan before providing services.
• Respond to coverage requests and confirm assignment details.
• Assist with bathing, dressing, grooming, and personal hygiene.
• Help clients move, transfer, and reposition safely.
• Perform prescribed mobility exercises according to the plan.
• Prepare meals, assist with household tasks, and offer companionship.
• Assist with transportation to appointments as assigned.
• Record services and communicate updates to the nurse case manager.
• Respect client preferences, dignity, and confidentiality.

Pay: $18.00 - $23.00 per hour

BrightStar Care of Fort Wayne is recruiting an experienced Home Health Aide (HHA) or Certified Nursing Assistant (CNA) for our Set Team in Fort Wayne, IN.

This role focuses on helping cover caregiver absences and other staffing needs. You’ll bring confidence, adaptability, and dependable care to different clients while staying connected with our scheduling and clinical teams.

Required On-Call Availability

• One designated day each week, 6 a.m.–11 p.m.
• One designated weekend each month
• Remain reachable during your assigned on-call periods
• Be available to accept client coverage during those periods
• Coordinate your assignments with the scheduling team

Scheduling options will be discussed around these coverage requirements.

How You’ll Support Our Clients

Your Home Health Aide (HHA) or Certified Nursing Assistant (CNA) experience will help you step into assignments, understand each client’s routine, and provide attentive care.
