Job summary

Elara Caring is hiring a LPN Clinical Coordinator in Tinley Park/Des Plaines, IL to coordinate home health services and support care teams.

You will review notes, obtain orders, monitor patient status, and assist with discharges and care planning. Travel up to 50% and direct patient contact may be required; strong nursing license and home care experience are essential.

Qualifications

• Graduate from an accredited school of professional nursing.
• Current LPN/LVN license in good standing.
• At least 1 year in home care or hospice settings.
• Willingness to travel up to 50% as needed.

Responsibilities

• Review and clear coordination notes and administrative tasks.
• Obtain and enter physician orders for CTM review.
• Monitor hospitalization lists and coordinate care resumption.
• Discharge patients from inpatient facilities and renew attachments.
• Participate in High-Risk Meetings and provide direct patient care when needed.
• Handle calls from patients and providers and relay messages.
• Provide scheduling, health summaries, and pre-authorizations support.

As a LPN Clinical Coordinator, you’ll contribute to our success in the following ways:

• Reviews and clears HCHB coordination notes and processes administrative task workflows in an accurate and timely manner.
• Obtains orders from physicians, including verbal, written and faxed orders, and enters into appropriate system for CTM review and approval.
• Monitors patient hospitalization list, confirms status, and coordinates appropriate resumption of care.
• Discharges patients in inpatient facility at end of episode (EOE) Obtains patient-related attachment renewal.
• Participate in High-Risk Meetings and ensure that the best provision of services is offered.
• Available to provide direct care, as needed.
• Answers telephone calls from patients, patient representatives, and medical professionals and communicate messages to the appropriate party or parties.
• Provides clerical support to patients such as but not limited to scheduling follow-up appointments for patients (as requested), providing health summaries to case managers, obtaining pre-authorizations, and addressing patient questions or concerns.

What is Required?

• Graduate of an accredited school of professional nursing
• Current Certified Licensed Vocational Nurse or Licensed Practical Nurse
• 1 year of home care or hospice experience
• Able to seek patient feedback, advocate for patients, understanding of patient needs, and managing patient expectations
• Able and willing to travel up to 50% as needed to support business and patient needs
• Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment
• Valid driver’s license and insurance and reliable transportation to perform job tasks

You will report to the Clinical Team Manager. The base pay for this position is $25 to $30 hourly, based on the company’s good faith estimate at the time of posting. Actual pay will be determined based on factors such as education, experience, skills, and internal equity.
