Job summary

Elara Caring seeks a LPN Clinical Coordinator to support home-based patient care in Des Plaines and Tinley Park. You will review HCHB notes, obtain physician orders, monitor hospitalization lists, and coordinate resumption or discharge of care while traveling up to 50% as needed.

The ideal candidate is a graduate of an accredited nursing program, licensed as an LPN, with 1 year of home care or hospice experience, and strong communication to advocate for patients.

Qualifications

• Graduate of an accredited school of professional nursing.
• Current LPN license (Licensed Practical Nurse or Vocational Nurse).
• 1 year of home care or hospice experience.
• Able to travel up to 50% as needed to support business and patient needs.
• Valid driver’s license and reliable transportation to perform job tasks.
• Ability to frequently lift, push, pull, and transfer patients up to 50 pounds.

Responsibilities

• Reviews and clears HCHB coordination notes and processes administrative task workflows.
• Obtains physician orders, verbal/written/faxed, and enters into CTM review system.
• Monitors hospitalization list, confirms status, coordinates resumption of care.
• Discharges patients in inpatient facility at end of episode (EOE).
• Obtains patient-related attachment renewals.
• Participates in High-Risk Meetings to ensure best provision of services.
• Available to provide direct care as needed.
• Handles calls from patients and professionals and relays messages.
• Provides clerical support: scheduling follow-ups, health summaries, pre-authorizations, and queries.

What is Required?* Graduate of an accredited school of professional nursing* Current Certified Licensed Vocational Nurse or Licensed Practical Nurse* 1 year of home care or hospice experience* Able to seek patient feedback, advocate for patients, understanding of patient needs, and managing patient expectations* Able and willing to travel up to 50% as needed to support business and patient needs* Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment* Valid driver’s license and insurance and reliable transportation to perform job tasks You will report to the Clinical Team Manager.*The base pay for this position is $25 to $30 hourly, based on the company’s good faith estimate at the time of posting. Actual pay will be determined based on factors such as education, experience, skills, and internal equity.*
