Job summary

Elara Caring is hiring a Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN) Clinical Coordinator to support in-home patient care and care coordination. You will operate within a collaborative team, review clinical notes, obtain physician orders, and help manage patient progression through care episodes.

The role requires a valid LPN/LVN license, at least 1 year of home care or hospice experience, and willingness to travel up to 50%.

Qualifications

• Graduate of an accredited school of professional nursing.
• Current Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN) license.

Responsibilities

• Reviews and clears HCHB coordination notes and processes administrative task workflows in an accurate and timely manner.
• Obtains orders from physicians, including verbal, written and faxed orders, and enters into appropriate system for CTM review and approval.
• Monitors patient hospitalization list, confirms status, and coordinates appropriate resumption of care.
• Discharges patients in inpatient facility at end of episode (EOE).
• Obtains patient-related attachment renewal.
• Participate in High-Risk Meetings and ensure that the best provision of services is offered.
• Available to provide direct care, as needed.
• Answers telephone calls from patients, patient representatives, and medical professionals and communicate messages to the appropriate party or parties.
• Provides clerical support to patients such as scheduling follow-up appointments, providing health summaries to case managers, obtaining pre-authorizations, and addressing patient questions or concerns.

As a LPN/LVN Clinical Coordinator, you’ll contribute to our success in the following ways:

What is Required?

• Graduate of an accredited school of professional nursing
• Current Certified Licensed Vocational Nurse or Licensed Practical Nurse
• 1 year of home care or hospice experience
• Able to seek patient feedback, advocate for patients, understanding of patient needs, and managing patient expectations
• Able and willing to travel up to 50% as needed to support business and patient needs
• Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment
• Valid driver's license and insurance and reliable transportation to perform job tasks

You will report to the Clinical Team Manager.
