Qualifications

• Legally authorized to work in the United States.
• Active clinical certification or licensure (MA/CMA, LPN, RN).
• Minimum 1 year clinical/administrative or care coordination experience.
• Proficient with Microsoft Teams, email, phone, and internal systems.
• Strong organizational and time-management skills.
• Knowledge of healthcare terminology and care coordination principles.

Responsibilities

• Coordinate patient-centered care activities in the EMR system.
• Support providers with documentation, follow-up tracking, and care coordination.
• Serve as a key communication link between providers and care teams.
• Relay patient needs and changes to the clinical team in a timely manner.
• Assist with referrals and coordination with external partners.
• Collaborate with coordinators and care team in an office-based setting.
• Maintain accurate documentation per workflows and escalation protocols.
• Contribute to team collaboration and daily office operations.
