Ennoble Care is seeking a full-time LPN Care Coordinator embedded in a partnered dialysis clinic in the Oklahoma City area. You will engage patients on-site, coordinate care plans with Ennoble providers and facility staff, and ensure timely follow-up and clear communication with patients and families.

The role emphasizes continuity of care, strong organization, and collaboration within a fast-paced clinical environment.

Qualifications

• Legally authorized to work in the United States
• Active, unrestricted LPN license in the state of Oklahoma
• Dialysis-specific certification (CDT, CHT, CCHT or CCHT-A) preferred
• Experience coordinating patient appointments within a clinical setting preferred
• Experience with hemodialysis/nephrology patients in a dialysis facility preferred
• Professional bedside manner with patient-centered approach
• Strong knowledge of healthcare terminology and care coordination principles
• Excellent organizational and time-management skills
• Proficient in Microsoft 365 tools (Outlook, Word, Excel) and Teams

Responsabilités

• Provide on-site engagement with dialysis patients to support care plans and follow-ups
• Coordinate care using the EMR to ensure continuity and timely follow-up
• Schedule patient visits for providers and support daily clinic workflow
• Collaborate with care providers and dialysis facility staff on patient-centered care
• Act as liaison between providers, patients, families and facility staff
• Communicate patient needs and changes in condition promptly to clinical leaders
• Maintain accurate documentation compliant with regulations
• Demonstrate professionalism and manage multiple priorities in a fast-paced clinic environment
• Support team collaboration and partnerships while maintaining organizational proficiency
