Job summary

Skyline Healthcare Center-San Jose is seeking an Occupational Therapy Assistant to support residents under the supervision of a licensed occupational therapist, following applicable federal guidelines and state practice acts.

The role involves resident screening, assisting with rehabilitative activities, monitoring progress, recording documentation, and coordination with staff to ensure quality care. AOTA-accredited associate degree and state-required OTA licensure are required or pending.

Qualifications

• Associates degree from an AOTA-accredited program is required.
• Licensed OTA if state law requires it.

Responsibilities

• Conducts resident screens (without interpretation) as allowed by state practice act.
• Helps residents with rehabilitative activities and exercises outlined in a treatment plan developed in collaboration with an occupational therapist.
• Monitors an individual’s activities to ensure correct performance and provide encouragement.
• Records resident’s progress for use by the occupational therapist.
• May assist in scheduling appointments, answering calls, restocking or ordering supplies, and completing insurance forms.
• Provides training to residents, caregivers and families according to the care plan.
• Completes required forms and documents per company policy and state/federal regulations.

Education

Associates Degree from a AOTA accredited program

Licensed OTA if required by state law
