Job summary

Innovate Rehab and Wellness is seeking a Licensed PRN Certified Occupational Therapy Assistant to support residents in a Skilled Nursing Facility. The role emphasizes one-on-one patient care, collaborative teamwork, and flexible scheduling to support work-life balance.

The COTA will implement OT plans, provide direct care under supervision, and document progress in the EMR while ensuring HIPAA and CMS compliance. Prior SNF experience is preferred; NBCOT and state license are required.

Qualifications

• Associate’s degree from OTA program required or equivalent.
• NBCOT certification and a valid state OTA license required.
• SNF or long-term care experience preferred.
• Excellent communication and interpersonal skills.

Responsibilities

• Implement treatment plans created by the Occupational Therapist.
• Provide direct patient care per state and facility guidelines.
• Monitor and document resident progress; report changes to OT.
• Educate residents and caregivers on adaptive techniques and equipment.
• Collaborate with nursing, PT, SLP, and other staff.
• Maintain accurate, timely documentation in the EMR.
• Participate in care planning and discharge planning.
• Ensure compliance with HIPAA, CMS, and all regulations.
