Job summary

Creative Solutions In HealthCare is seeking a Per Diem Occupational Therapy Assistant (OTA) to join our in-house rehab team. You will deliver patient-centered OT services in SNF/long-term care settings on a PRN basis, under supervision and per physician orders.

You’ll contribute to individualized plans, document care thoroughly, and collaborate with the Rehab Director and care team to promote independence and quality of life for residents.

Qualifications

• Active and valid Texas Occupational Therapy Assistant license.
• COTA credential required.
• In good standing with licensing boards.
• SNF or long-term care experience preferred.
• Knowledge of current therapy practices and documentation.

Responsibilities

• Plan and administer therapies to residents to relieve pain and improve function.
• Provide OT services under supervision of an OT per physician orders.
• Relay service requests to the OT and respond promptly.
• Ensure treatments follow the care plan.
• Provide instruction to patients, families, and caregivers.
• Monitor responses and report changes to the OT.
• Complete documentation per policy and regulations.
• Attend required Rehab Director meetings.

Join Our Team as a Per Diem (PRN) Occupational Therapy Assistant (OTA)

We are seeking a compassionate and motivated Occupational Therapy Assistant (OTA) to join our in‑house team of outstanding professionals on a PRN basis. In this role, you will provide person‑centered care to help residents achieve positive outcomes and improve their independence and quality of life.

Your Impact as an Occupational Therapy Assistant (OTA)

• Plan and administer occupational therapy treatments to residents and patients with temporary or long‑term disabilities to relieve pain, restore or improve function, and promote healing
• Provide skilled occupational therapy services and interventions in accordance with physician orders under the supervision of an Occupational Therapist
• Respond to requests for service by relaying information and referrals to the Occupational Therapist
• Ensure all treatments are delivered in accordance with the established plan of care
• Provide clinical support and instruction to patients, families, and caregivers
• Monitor patient response to treatment interventions and report changes as appropriate
• Complete required documentation thoroughly and timely in accordance with company policy and state/federal regulations
• Attend required meetings as designated by the Director of Rehab

What Makes You a Great Fit

• Active and valid Texas Occupational Therapy Assistant license
• Certified Occupational Therapy Assistant (COTA) credential required
• In good standing with all regulatory agencies and licensing boards
• Skilled Nursing Facility (SNF) or long‑term care experience preferred
• Current knowledge of occupational therapy treatment practices
• Experience with Net Health Electronic Documentation System preferred
• Ability to manage patients with varying personalities and needs
• Strong communication and teamwork skills
