Job summary

Creative Solutions In HealthCare is seeking a compassionate Per Diem Occupational Therapy Assistant (OTA) to join our in-house rehab team. You will plan and administer treatments to residents with temporary or long-term disabilities, under supervision of an OT, to relieve pain and improve function.

Responsibilities include documenting thoroughly, communicating with the OT, families, and care staff, and ensuring care plans are followed.

Qualifications

• Active and valid Texas Occupational Therapy Assistant license.
• COTA credential required.
• In good standing with regulatory agencies and licensing boards.
• SNF or long-term care experience preferred.
• Knowledge of occupational therapy treatment practices.

Responsibilities

• Plan and administer occupational therapy treatments to residents to relieve pain, restore or improve function, and promote healing.
• Provide skilled occupational therapy services and interventions under the supervision of an Occupational Therapist.
• Respond to requests for service by relaying information and referrals to the Occupational Therapist.
• Ensure all treatments are delivered in accordance with the established plan of care.
• Provide clinical support and instruction to patients, families, and caregivers.
• Monitor patient response to treatment interventions and report changes as appropriate.
• Complete required documentation thoroughly and timely in accordance with company policy and state/federal regulations.
• Attend required meetings as designated by the Director of Rehab.

We are seeking a compassionate and motivated Occupational Therapy Assistant (OTA) to join our in‑house team of outstanding professionals on a PRN basis. In this role, you will provide person‑centered care to help residents achieve positive outcomes and improve their independence and quality of life.

Your Impact as an Occupational Therapy Assistant (OTA)

• Plan and administer occupational therapy treatments to residents and patients with temporary or long‑term disabilities to relieve pain, restore or improve function, and promote healing
• Provide skilled occupational therapy services and interventions in accordance with physician orders under the supervision of an Occupational Therapist
• Respond to requests for service by relaying information and referrals to the Occupational Therapist
• Ensure all treatments are delivered in accordance with the established plan of care
• Provide clinical support and instruction to patients, families, and caregivers
• Monitor patient response to treatment interventions and report changes as appropriate
• Complete required documentation thoroughly and timely in accordance with company policy and state/federal regulations
• Attend required meetings as designated by the Director of Rehab

What Makes You a Great Fit

• Skilled Nursing Facility (SNF) or long‑term care experience preferred

• Experience with Net Health Electronic Documentation System preferred
• Ability to manage patients with varying personalities and needs
