Responsibilities

• Follows the therapist's plan of care and communicates with the evaluating therapist.
• Implements patient-centered, occupation-based intervention plans and adjusts as needed.
• Demonstrates sequential treatment planning considering patient and family needs.
• Adapts activities for physical, cognitive, behavioral limitations.
• Collaborates with patient, family, and others; educates patients and caregivers.
• Produces clear documentation and communicates with referring providers.
• Participates in quality of service delivery initiatives.
• Completes daily documentation in EMR and charging in therapy records.
• Provides ROM measurements of the elbow, wrist, and hand.
• Completes training for ADLs including dressing, cooking skills.
