Continuum Therapy Partners, LLC is seeking a Certified Occupational Therapy Assistant to join our therapist-led team in Wisconsin. The role focuses on implementing individualized treatment plans under the supervision of an Occupational Therapist and documenting patient progress.

The positions are part-time daytime at Meadowview Health Services in Sheboygan and Plymouth Health Services in Plymouth, WI. Ideal candidates hold an Associate Degree, current COTA license, NBCOT certification, and

• Associate degree in Occupational Therapy Assistant required.
• Current, unencumbered state COTA license; NBCOT certification required.
• 1 year of experience as a Certified Occupational Therapy Assistant preferred.
• Implements individualized treatment plans under the supervision of an Occupational Therapist.
• Documents patient progress and reports changes in patient status to the supervising OT.
• Educates patients on adaptive equipment and home exercise programs.
