Concept Rehab is seeking a dedicated Certified Occupational Therapy Assistant (COTA) to join our rehabilitation team in a skilled nursing and post-acute care setting. You will work with Occupational Therapists and interdisciplinary teams to help patients regain independence, improve function, and enhance safety in daily activities.

Qualifications
- Graduate of an accredited Occupational Therapy Assistant program.
- Active Ohio COTA license in good standing.
- Skilled nursing, geriatric, or post-acute care experience preferred.
- Strong interpersonal and communication skills.
- Ability to work effectively within a multidisciplinary healthcare team.
- Compassionate, patient-focused approach to care.

Responsibilities
- Implement individualized treatment plans designed to improve functional performance.
- Provide skilled occupational therapy interventions that support independence with daily activities.
- Assist patients with self-care, dressing, grooming, bathing, and other ADLs.
- Support upper extremity strengthening, fine motor coordination, balance, and functional mobility activities.
- Monitor patient progress and communicate changes to supervising therapists.
- Collaborate with nursing staff, rehabilitation team members, patients, and families.
- Educate caregivers and facility staff on therapeutic techniques and safety recommendations.
- Contribute to discharge planning and continuity of care efforts.
- Maintain timely, accurate, and compliant electronic documentation.
- Promote a positive and engaging rehabilitation experience for patients.
