Concept Rehab, Inc. is seeking a dedicated Certified Occupational Therapy Assistant (COTA) to join our rehabilitation team in Cincinnati, OH in a skilled nursing and post-acute setting.

This full-time position also offers PRN options to support patient care and continuity of therapy services. You will work with Occupational Therapists and the care team to improve function, safety, and independence, delivering high-quality, patient-centered care and contributing to discharge planning and

Qualifications

• Graduate of an accredited Occupational Therapy Assistant program.
• Active Ohio COTA license in good standing.
• Skilled nursing, geriatric, or post-acute care experience preferred.
• Strong interpersonal and communication skills.
• New graduates encouraged to apply.

Responsibilities

• Implement individualized treatment plans designed to improve functional performance.
• Provide skilled occupational therapy interventions that support independence with daily activities.
• Assist patients with self-care, dressing, grooming, bathing, and other activities of daily living (ADLs).
• Support upper extremity strengthening, fine motor coordination, balance, and functional mobility activities.
• Monitor patient progress and communicate changes to supervising therapists.
• Collaborate with nursing staff, rehabilitation team members, patients, and families.
• Educate caregivers and facility staff on therapeutic techniques and safety recommendations.
• Contribute to discharge planning and continuity of care efforts.
• Maintain timely, accurate, and compliant electronic documentation.
• Promote a positive and engaging rehabilitation experience for patients.
