Concept Rehab in Cincinnati, OH is seeking a dedicated Certified Occupational Therapy Assistant (COTA) to join our skilled nursing and post-acute rehabilitation team. You’ll work closely with Occupational Therapists and care teams to help patients improve function, safety, and independence in daily activities.

Qualifications

• Graduate of an accredited Occupational Therapy Assistant program.
• Active Ohio COTA license in good standing.
• Geriatric or post-acute care experience preferred.

• Ability to work effectively within a multidisciplinary healthcare team.

Responsibilities

• Implement individualized treatment plans to improve functional performance.
• Provide skilled OT interventions to support independence in ADLs.
• Assist patients with self-care, dressing, grooming, bathing, and other ADLs.
• Support upper extremity strengthening, fine motor coordination, balance, and mobility.
• Monitor progress and communicate changes to supervising therapists.
• Collaborate with nursing staff and rehabilitation team members.

Requirements

What You'll Do

• Implement individualized treatment plans designed to improve functional performance
• Provide skilled occupational therapy interventions that support independence with daily activities
• Assist patients with self-care, dressing, grooming, bathing, and other activities of daily living (ADLs)
• Support upper extremity strengthening, fine motor coordination, balance, and functional mobility activities
• Monitor patient progress and communicate changes to supervising therapists
• Collaborate with nursing staff, rehabilitation team members, patients, and families
• Educate caregivers and facility staff on therapeutic techniques and safety recommendations
• Contribute to discharge planning and continuity of care efforts
• Maintain timely, accurate, and compliant electronic documentation
• Promote a positive and engaging rehabilitation experience for patients

Qualifications

• Graduate of an accredited Occupational Therapy Assistant program
• Active Ohio COTA license in good standing
• Skilled nursing, geriatric, or post-acute care experience preferred
