Job summary

Empath Health is seeking a compassionate Home Health COTA to deliver individualized occupational therapy services to patients in their homes in Naples, FL and surrounding areas. The role involves working Monday through Friday, 40 hours per week, with supervision and collaboration within the home health team.

You will provide OT services per plan of care, assess equipment needs to improve independence, and document progress notes for timely submission.

Qualifications

• Active Florida COTA license.
• Minimum of one year experience as a licensed Occupational Therapy Assistant.
• Home health care experience preferred.

Responsibilities

• Provide occupational therapy services per the plan of care under supervision of the OT/Director.
• Evaluate equipment needs to increase the patient’s functional level.
• Prepare clinical and progress notes of services provided and submit in a timely manner.
• Collaborate with the interdisciplinary team to ensure seamless, holistic care.
