Job summary

CommonSpirit Health at Home in Kendallville, IN is hiring an Occupational Therapy Assistant to join our Parkview Health at Home team. The OTA will work with the OT and home health staff to plan, implement, and evaluate services for patients in their homes.

We offer competitive wages, an 8-hour day shift, and mileage reimbursement (58.5 cents per mile) with no after-hours on-call requirements. Prior home health experience is preferred and CPR/BLS certification is required.

Qualifications

• AAS degree in OTA studies or related field.
• Active registered OTA license in the state of practice.
• Must obtain and maintain CPR/BLS Certification.
• One-year OTA experience in a clinical setting.
• Current driver's license.
• Comply with federal, state and local laws and regulations.

Responsibilities

• Assess patients’ physical, cognitive, and psychosocial abilities to determine therapy needs. Develop and implement personalized treatment plans that may include exercises, adaptive techniques, and use of assistive devices.
• Teach patients and family members about home safety, adaptive equipment, and strategies to manage daily activities.
• Maintain accurate and timely records of patient progress, treatment plans, and modifications to therapy programs for compliance and regulatory standards.
• Monitor patients progress and adjust treatment plans as necessary to meet changing needs. Utilize outcome-based measures to assess effectiveness.
• Collaborate and communicate patient’s change in condition with physicians, nurses, physical therapists, speech therapists, and other home health team members.

Job Requirements

• Associates of applied science (AAS) degree in occupational therapy assistant studies or a related field.
