Job summary

Boost Home Healthcare seeks an Occupational Therapist to deliver high-quality home health OT services. The role focuses on improving patients’ function and independence in their own homes with personalized care plans and timely reassessments.

You will work with a multidisciplinary team, coordinate with physicians, and provide skilled OT procedures while adhering to regulatory standards. Prior home health experience is preferred; licensure requirements apply.

Qualifications

• Graduate of an Occupational Therapy program accredited by AMA/AOTA.
• Eligible for NBCOT certification or already certified.
• Licensed to practice OT in the practicing state.

Responsibilities

• Improve or minimize residual disabilities and restore productive living.
• Develop and maintain plan of care; reassess every 30 days.
• Provide prescribed occupational therapy and skilled procedures.
• Initial assessment and care plan development; coordinate with physicians.
• Collaborate with other home care staff in patient care planning.

What you’ll be doing:

• Improve or minimize residual physical disabilities of the patient.
• Return the individual to optimum and productive living within the patient’s capability.
• Periodically participate with all other home care personnel in patient’s care planning.
• Provide prescribed occupational therapy and all skilled procedures.
• Responsible for initial assessment, plan of care maintenance program development and modifications and reassessments every 30 days.
• Consult with physicians regarding change in treatment.

Compensation and Schedule:

• $38-$45 per visit
• Monday through Friday
• Weekends as needed
• Part time and PRN

What we’re looking for:

• A passion to serve and help others live their best lives possible.
• Graduate of an Occupational Therapy curriculum accredited by the Committee on Allied Health Education and Accreditation of the American Medical Association and the American Occupational Therapy Association (AOTA) and /or certified by successfully completing the National Certification examination, -or-
• Eligible for the National Registration Examination of the American Occupational Therapy Association
• Currently licensed by the Board of Occupational and Physical Therapy Examiners in the state in which practicing, unless licensure does not apply
• Minimum of one year in an acute care setting.
• One (1) year experience in Home Health preferred.
