Job summary

Boost Home Healthcare is seeking passionate Occupational Therapists to join our family-like culture in Greeley, Colorado. You will be responsible for enhancing patients' quality of life through personalized home healthcare.

The ideal candidate will have a degree from an accredited Occupational Therapy program, a current state license, and some home health experience is preferred. We offer a flexible part-time schedule with competitive pay ranging from $38 to $45 per visit.

• Minimum of one year in an acute care setting.

Responsibilities

• Improve or minimize residual physical disabilities of the patient.
• Return the individual to optimum living within their capability.
• Provide prescribed occupational therapy procedures.
• Perform initial assessments and plan of care modifications.

What you’ll be doing

• Improve or minimize residual physical disabilities of the patient.
• Return the individual to optimum and productive living within the patient’s capability.
• Periodically participate with all other home care personnel in patient’s care planning.
• Provide prescribed occupational therapy and all skilled procedures.
• Responsible for initial assessment, plan of care maintenance program development and modifications and reassessments every 30 days.
• Consult with physicians regarding change in treatment.

Compensation and Schedule

• $38-$45 per visit
• Monday through Friday
• Weekends as needed
• Part time and PRN

What we’re looking for

• A passion to serve and help others live their best lives possible.
• Graduate of an Occupational Therapy curriculum accredited by the Committee on Allied Health Education & Accreditation of the American Medical Association and the American Occupational Therapy Association (AOTA) and /or certified by successfully completing the National Certification examination, -or-
• Eligible for the National Registration Examination of the American Occupational Therapy Association
• Currently licensed by the Board of Occupational and Physical Therapy Examiners in the state in which practicing, unless licensure does not apply
• Minimum of one year in an acute care setting.
• One (1) year experience in Home Health preferred.
