Position Summary

Assists in providing rehabilitative services to residents with a range of mental, physical, emotional, and developmental needs, supporting their independence and quality of life. This role is as much about building trust and genuine connection with residents and colleagues as it is about clinical skill - we believe every person, team member and resident alike, brings unique strengths, perspectives, and life experience that make our community stronger.

Essential Duties
  • Provides services under the direction of a licensed occupational therapist, working as a collaborative partner within the broader care team.
  • Follows treatment plans developed in collaboration with the occupational therapist by teaching techniques such as the proper method of moving from a bed into a wheelchair and the best way to stretch and limber muscles.
  • Evaluates the living skills and capabilities of physically, developmentally, and emotionally disabled residents, taking time to understand each individual's personality, preferences, and goals - not just their diagnosis.
  • Assists in the selection of activities and the development of treatment plans to help residents function independently, tailoring approaches to what makes each resident feel respected and motivated.
  • Builds authentic, trust-based relationships with residents and families, creating a warm and welcoming environment where people feel seen and valued.
  • Observes, monitors, and records patient progress, attitudes, and behavior, providing thoughtful feedback to superiors and recommending changes to treatment plans.
  • Instructs residents and their families on in-home programs, basic living skills, and the care and use of adaptive equipment, communicating with patience and empathy.
  • Performs routine tasks such as helping residents perform stretches and teaching residents to use specific equipment.
  • Uses a variety of tools and equipment, such as special lifts or pulleys, to safely move residents.
  • Documents patient care services according to approved medical and facility documentation procedures.
  • Collaborates closely with fellow team members, contributing to a supportive, inclusive workplace culture where diverse backgrounds and perspectives are welcomed and celebrated.
