PRN Home Health & Therapy in Green Bay is seeking a full-time Occupational Therapist to join our growing team, with 0.8–1.0 FTE and the freedom to manage your own schedule. You’ll deliver compassionate home health care to older adults across Green Bay and nearby communities, building meaningful patient relationships.

• Review and respond to patient referrals in a timely manner.
• Collaborate with patients, caregivers, and interdisciplinary team members to develop individualized care plans.
• Create and implement treatment plans with measurable, patient-centered functional goals.
• Educate patients and caregivers to support successful outcomes and maximize independence.
• Provide guidance, direction, and supervision to Certified Occupational Therapy Assistants (COTAs).
• Assess and recommend durable medical equipment (DME), including wheelchairs, adaptive equipment, and other assistive devices.
• Partner with healthcare professionals to ensure coordinated, high-quality patient care.

In this role, you'll primarily provide care in a home health setting, with occasional visits to skilled nursing facilities. Whether you're an experienced clinician or a new graduate eager to begin a rewarding career in home health, this position offers an excellent opportunity to grow and succeed.
