Continuum Therapy Partners is seeking Occupational Therapists to join our home health team serving Stevens Point, Wisconsin and surrounding areas.

This role offers a flexible, full-time daytime schedule with PRN options and the opportunity to deliver meaningful one-on-one patient care within a supportive, therapist-led culture.

Qualifications

• Master’s or Doctorate Degree in Occupational Therapy required.
• Current, unencumbered state Occupational Therapy License; NBCOT Certification required.
• 1 year of previous experience as an Occupational Therapist preferred.
• Evaluates and develops individualized treatment plans to improve patients’ functional independence.
• Documents patient progress and modifies treatment plans based on clinical outcomes.
• Educates patients and families on adaptive equipment and home safety recommendations.
