Job summary

Continuum Therapy Partners in Stevens Point, WI is seeking a Home Health Occupational Therapist to deliver compassionate, patient-centered care in clients’ homes. This role offers a flexible schedule and opportunities for growth within a therapist-led organization.

Qualifications

• Master's or Doctorate Degree in Occupational Therapy.
• Current Wisconsin OT license and NBCOT certification.
• 1 year of OT experience preferred.

Responsibilities

• Evaluates and develops individualized treatment plans to improve patients' functional independence.
• Documents patient progress and modifies treatment plans based on outcomes.
• Educates patients and families on adaptive equipment and home safety.

Occupational Therapist Requirements & Responsibilities:

• Master's or Doctorate Degree in Occupational Therapy required
• Current, unencumbered state Occupational Therapy License; NBCOT Certification required
• 1 year of previous experience as an Occupational Therapist preferred
• Evaluates and develops individualized treatment plans to improve patients' functional independence
• Documents patient progress and modifies treatment plans based on clinical outcomes
• Educates patients and families on adaptive equipment and home safety recommendations
