Morristown Post Acute is seeking an Occupational Therapist (OT) to join our team in Morristown, NJ. The OT will assess residents’ functional abilities with emphasis on ADLs and IADLs, and develop individualized plans to improve independence.

The role involves performing comprehensive evaluations, administering OT interventions, educating residents and families, and documenting care in EMRs while collaborating with physical therapists and speech-language pathologists to support holistic care.

Responsabilidades

• Assessment and evaluation of residents' functional abilities focusing on ADLs and IADLs.
• Develop tailored intervention plans to improve independence in daily activities.
• Collect Section GG data for Annual, Quarterly, Falls and other assessments and clinical data as needed.
• Administer OT interventions including ADL training, adaptive equipment provision, and environmental modifications.
• Educate residents, families and care staff on safe mobility techniques and exercises; conduct RTM check-ins post-discharge.
• Maintain detailed documentation of resident evaluations, treatment plans, progress notes and EMR updates.
• Collaborate with rehab staff, including PTs and SLPs, for holistic resident care.

Responsabilities:

• Assessment and Evaluation: Perform comprehensive assessments of residents' functional abilities, with a focus on activities of daily living (ADLs) and instrumental activities of daily living (IADLs).
• Individualized Care Plans: Develop tailored intervention plans to improve residents' independence in daily activities.
• Section GG Data Collection: Ensure accurate and consistent collection of Section GG data for Annual, Quarterly, Falls, Significant Change assessments, BIMS, PHQ 2-9, and other clinical data as needed.
• Treatment: Administer OT interventions, including ADL training, adaptive equipment provision, and environmental modifications.
• Education: Educate residents, family members, and care staff on safe mobility techniques and exercises for continued progress. Conducts remote check-in through remote therapeutic monitoring(RTM) with discharged patients to home or next level of care.
• Documentation: Comfortable and knowledgeable with Point of Service( PoS) care delivery and documentation. Maintain detailed and up-to-date documentation of resident evaluations, treatment plans, daily and progress notes, HMO/Manage care progress updates in EMRs( Net Health/PCC).
• Interdisciplinary Collaboration: Collaborate with other rehab staff, including Physical Therapists and Speech-Language Pathologists, for holistic resident care.
