Job summary

Morristown Post Acute in Morristown, NJ is seeking a Physical Therapist to join our team. The role supports the Physical Therapist, implements treatment plans, educates residents and families, and collaborates with an interdisciplinary rehab team.

Responsibilities include monitoring patient progress, accurate documentation in EMRs, and remote check-ins for discharged patients as part of RTM, with a focus on safe mobility techniques and ongoing care pathways.

Qualifications

• Experience assisting Physical Therapist and carrying out treatment plans.
• Ability to document resident progress and interventions.
• Provide education on mobility techniques to residents and staff.

Responsibilities

• Assist the Physical Therapist in carrying out treatment plan and gather objective data for Section GG collection during Annual, Quarterly, Falls, and Significant Change screenings.
• Treatment Support: Implement treatment plans as directed by the Physical Therapist, including exercises, modalities, and mobility training.
• Monitoring: Observe residents' performance and report any changes or concerns to the supervising Physical Therapist.
• Documentation: Maintain accurate and timely documentation of resident progress and any interventions provided. Comfortable and knowledgeable with Point of Service( PoS) care delivery and documentation. Maintain detailed and up-to-date documentation of resident evaluations, treatment plans, daily and progress notes, HMO/Manage care progress updates in EMRs( Net Health/PCC).
• Education: Educate residents, family members, and care staff on safe mobility techniques and exercises for continued progress. Conducts remote check-in through remote therapeutic monitoring(RTM) with discharged patients to home or next level of care.
• Team Collaboration: Collaborate with the interdisciplinary team, following the guidance of the Physical Therapist and in cooperation with other rehab staff.
