Job summary

Evolve Therapy Services in South Florida is seeking an Occupational Therapist for an in-house Skilled Nursing Facility. This is a full-time role with a 6-month contract-to-permanent path and relocation assistance.

You will assess residents, develop and implement treatment plans, coordinate with a multidisciplinary team, supervise a COTA, and ensure compliant documentation. This position offers growth and Florida sunshine benefits.

Qualifications

• Degree in Occupational Therapy.
• Active Occupational Therapy license in practicing state(s).
• Ongoing continuing education and professional development to maintain licensure and certification.

Responsibilities

• Screen regular residents to determine the best intervention or treatment for each patient.
• Evaluate residents to inform and develop treatment plans; establish short- and long-term goals and methods to achieve goals.
• Interpret and communicate evaluation findings, treatment plans, and recommendations to residents, families/caregivers, and interdisciplinary team members.
• Establish individual resident therapy schedules in cooperation with other team members.
• Conduct therapy according to established treatment plan and ensure proper documentation and billing of occupational therapy services.
• Perform all other duties as assigned and as required to effectively treat patients.
• Oversee COTA as needed and ensure all documentation and treatments are carried out with clinical excellence and proper reimbursement practices.
