Job summary

Nexus at Alton is seeking an Occupational Therapist to evaluate and treat residents, helping them regain or improve daily living skills for independence. You will develop individualized plans and work with an interdisciplinary team to optimize outcomes.

Requires an active IL OT license and preferred SNF/LTC experience. The role emphasizes thorough documentation and adherence to state and federal regulations within a collaborative, supportive culture.

Qualifications

• Active Illinois license in Occupational Therapy.
• SNF/Long-term care experience preferred.
• Knowledge of current treatment practices.
• Knowledge of PCC Electronic Documentation System preferred.

Responsibilities

• Develop personalized treatment plans to maximize safety and performance in ADL and IADL.
• Ensure all treatment delivered aligns with the plan of care.
• Monitor patient response and adjust treatment with IDT collaboration.
• Complete documentation thoroughly and in line with regulatory requirements.
