Job summary

Enhance Rehabilitation is seeking a qualified Occupational Therapist to join our dynamic SNF team in Granite City, IL. You will evaluate and treat patients, develop plans of care, and set measurable functional goals within a collaborative care model.

You will also re-evaluate progress, adjust treatment plans, and document evaluations, discharges, and daily notes while supervising Occupational Therapy Assistants to ensure exceptional patient outcomes.

Qualifications

• Licensed Occupational Therapist with SNF experience.
• Evaluates and treats patients per individualized plan of care.
• Coordinates with facility staff to ensure high-quality patient outcomes.

Responsibilities

• Provide patient evaluations and develop plans of care with measurable goals.
• Treat patients and educate them on injury, progress and outcomes.
• Re-evaluate progress and adjust plans of care and goals.
• Complete timely documentation: evaluations, re-evaluations, discharges and daily progress notes.
• Supervise Occupational Therapy Assistants.
