Job summary

Enhance Rehabilitation in Illinois is seeking qualified Occupational Therapist candidates to join our team in a SNF setting, evaluating and treating patients according to individualized plans of care.

You will collaborate with facility staff to ensure excellent care, develop plans of care with measurable goals, educate patients and families, and complete timely documentation. You will also supervise Occupational Therapy Assistants as part of a collaborative team.

Responsibilities

• Evaluating new patients, developing plans of care and setting measurable, functional goals
• Treating patients, answering their concerns and educating them on their injury, progress and probable outcomes
• Re-evaluating patient progress, revising plans of care and resetting goals
• Complete timely and accurate documentation including; evaluations, re-evaluations discharges and daily progress notes.
• Supervising Occupational Therapy Assistants.
