Enhance Rehabilitation in Illinois is seeking an Occupational Therapist to join our SNF team. You will evaluate and treat patients, develop plans of care, and set measurable goals to improve function.

You will re-evaluate progress, adjust plans, document evaluations and daily notes, and supervise Occupational Therapy Assistants while collaborating with facility staff to ensure excellent patient care and outcomes.

Qualifications

• Evaluate patients and develop plans of care with measurable functional goals.
• Treat patients and educate them on injury, progress and outcomes.
• Re-evaluate progress and revise plans of care to reset goals.
• Complete timely documentation including evaluations, re-evaluations, discharges and daily progress notes.
• Supervise Occupational Therapy Assistants.
• Collaborate with facility staff to ensure excellent patient care.

Responsabilités

• Collaborate with facility staff to ensure excellent care for your patients
