Job summary

Nexus Health is seeking an Occupational Therapist to evaluate and treat residents, helping them regain or improve daily living skills and functional independence. Missouri-licensed therapists are welcome, and we cover your Illinois reciprocity license costs.

Responsibilities include developing individualized treatment plans, ensuring plans of care are followed, and documenting thoroughly to meet regulatory standards. SNF exposure and PCC system familiarity are preferred.

Qualifications

• Active Illinois license in Occupational Therapy (must be currently valid).
• SNF/long-term care experience preferred.
• Knowledge of PCC Electronic Documentation System preferred.

Responsibilities

• Develop personalized treatment plans to maximize safety and independence in ADLs and IADLs.
• Ensure all treatment aligns with the established plan of care.
• Monitor patient response and modify treatment with IDT collaboration as needed.
• Ensure timely and thorough documentation in compliance with regulatory requirements.
