Job summary

Evolve Therapy Services, LLC in Olivia, MN seeks an Occupational Therapist to provide in-house SNF therapy with a focus on quality resident care.

Full Time, Part Time, or PRN positions available. The role requires an OT degree and an active license; there is a $5,000 sign-on bonus plus tuition reimbursement and opportunities for growth within the organization.

Qualifications

• Degree in Occupational Therapy.
• Active Occupational Therapy license in practicing state(s).
• Ongoing continuing education and professional development to maintain licensure and certification.

Responsibilities

• Conduct regular screening of residents to determine the best intervention for each patient.
• Evaluate residents to inform treatment plans and set short- and long-term goals.
• Interpret and communicate evaluation findings, treatment plans, and recommendations to residents, families/caregivers, and interdisciplinary team members.
• Establish individual resident therapy schedules in cooperation with other team members.
• Conduct therapy according to established treatment plan and ensure proper documentation and billing of occupational therapy services.
• Perform other duties as assigned to effectively treat patients.
• Oversee COTA as needed and ensure documentation and treatments are carried out with clinical excellence and proper reimbursement practices.

Education

Degree in Occupational Therapy

Active Occupational Therapy license in practicing state(s)

Ongoing continuing education and professional development to maintain licensure and certification
