Job summary

Evolve Therapy Services, LLC is seeking a dedicated Occupational Therapist for an in-house Skilled Nursing Facility in Olivia, MN. Full-time, part-time, and PRN positions are available, with a $5,000 sign-on bonus and tuition reimbursement.

You will plan, organize, develop, and direct occupational therapy services in accordance with standards and directives, ensuring high-quality resident care at all times.

Qualifications

• OT degree required.
• Active OT license required in practicing state(s).
• Ongoing continuing education to maintain licensure/certification.

Responsibilities

• Screen residents to determine best OT interventions for each patient.
• Evaluate residents and develop individualized treatment plans with short- and long-term goals.
• Communicate findings and plans to residents, families, and interdisciplinary team members.
• Set up resident therapy schedules with other team members.
• Provide therapy according to plan and ensure proper documentation and billing.
• Perform additional duties to ensure high-quality patient care.
• Oversee COTA and ensure compliant documentation and reimbursement practices.

Education

Degree in Occupational Therapy

Active Occupational Therapy license in practicing state(s)
