Job summary

Evolve Therapy Services, LLC is seeking an Occupational Therapist to join our in-house Skilled Nursing Facility team in Franklin, MN. You will plan, direct, and deliver OT services to residents in accordance with federal, state, and local standards, collaborating with a multidisciplinary team.

This full-time, part-time, or PRN position offers a $5,000 sign-on bonus and tuition reimbursement, with competitive pay and opportunities for growth.

Qualifications

• Degree in Occupational Therapy.
• Active Occupational Therapy license in practicing state(s).
• Ongoing continuing education to maintain licensure and certification.

Responsibilities

• Screen residents to determine the best intervention for each patient.
• Evaluate residents to inform treatment plans; establish short- and long-term goals.
• Interpret and communicate evaluation findings, treatment plans, and recommendations to residents, families/caregivers, and interdisciplinary team members.
• Establish individual resident therapy schedules in cooperation with other team members.
• Conduct therapy according to established treatment plan and ensure proper documentation and billing of occupational therapy services.
• Perform all other duties as assigned and as required to effectively treat patients.
• Oversee COTA as needed and ensure all documentation and treatments are carried out with clinical excellence and proper reimbursement practices are being followed.
