Enhance Rehabilitation in Illinois is seeking a qualified Occupational Therapist to join our growing team in a SNF setting. In this role, you will evaluate and treat patients following an individualized plan of care, and collaborate with facility staff to ensure high-quality care.

You will evaluate new patients, develop plans of care with measurable goals, treat patients, address concerns, and educate them on progress and outcomes.

• Collaborate with facility staff to ensure excellent care for your patients.
• Evaluating new patients, developing plans of care and setting measurable goals.
• Treating patients, answering their concerns and educating them on their injury, progress and probable outcomes.
• Re-evaluating patient progress, revising plans of care and resetting goals.
• Complete timely and accurate documentation including; evaluations, re-evaluations discharges and daily progress notes.
• Supervising Occupational Therapy Assistants.
