Enhance Rehabilitation is seeking an Occupational Therapist to join our team in Crystal Lake, IL. You will evaluate and treat patients in a skilled nursing facility setting, following an individualized plan of care.

You will collaborate with facility staff, develop and adjust plans of care and set measurable, functional goals while providing patient education and ensuring high-quality documentation. Supervising Occupational Therapy Assistants and maintaining excellent patient care are essential

Responsabilités

• Evaluating new patients, developing plans of care and setting measurable, functional goals
• Treating patients, educating them on injury, progress and probable outcomes
• Re-evaluating patient progress, revising plans of care and resetting goals
• Complete timely and accurate documentation including evaluations, re-evaluations, discharges and daily progress notes
• Supervising Occupational Therapy Assistants
• Collaborating with facility staff to ensure excellent patient care
