Job summary

(Manna Post Acute) Pickens Skilled Nursing, LLC is seeking a compassionate Occupational Therapist (OTR) to join our rehabilitation team in Pickens, SC. You will evaluate residents, develop individualized plans, and deliver therapy to maximize independence, safety, and quality of life.

The role requires a Bachelor's in OT; a Master's or Doctorate is preferred. Experience in skilled nursing or long-term care is valued, with PCC and Casamba experience a plus.

Qualifications

• Bachelor's degree in Occupational Therapy is required.
• Master's or Doctorate in Occupational Therapy is preferred.
• Experience in skilled nursing/rehabilitation/long-term care is preferred.
• Experience with PCC and Casamba is preferred.

Responsibilities

• Evaluate residents and develop individualized occupational therapy treatment plans.
• Provide skilled OT services per physician orders and goals.
• Monitor progress and modify treatment plans as needed.
• Communicate with physicians, nursing staff, residents, families and interdisciplinary team.
• Maintain accurate documentation per regulatory and payer requirements.
• Participate in care planning meetings and discharge planning activities.
• Educate residents and families on treatment goals and home programs.
• Conduct staff training on OT topics as needed.
• Ensure compliance with state acts, policies and regulatory requirements.
• Promote resident safety, independence and quality of life.
