Job summary

Sweetwater Care is seeking a dedicated Occupational Therapist to join our in-house therapy team at our family-owned facility in California. The OT will assess patients with cognitive and ADL/physical functioning needs, develop care plans, and provide therapeutic interventions while ensuring regulatory compliance.

Supervision of assistants and coordination with the rehabilitation team are essential. We offer a flexible schedule with opportunities for full-time, part-time, or PRN arrangements,

Qualifications

• Active State Licensure as an Occupational Therapist.
• Adherence to NBCOT/AOTA and state practice acts.

Responsibilities

• Screen/evaluate patients with cognitive or ADL/physical functioning disorders and develop plans of care.
• Use standardized assessments and evidence-based practice.
• Provide rehabilitative treatment per regulatory standards.
• Document progress and outcomes in the patient record.
• Collaborate with supervisor and health team members and communicate patient progress.
