Job summary

Sweetwater Care and Rehab invites an Occupational Therapist to join our in-house therapy team whose focus is patient-centered care and collaborative problem solving with the IDT. Responsibilities include providing rehabilitative therapy for cognitive and ADL functioning, documenting outcomes, supervising aides, and contributing to care plans and discharge summaries.

Active State Licensure, NBCOT/AOTA adherence, and professional ethics are required; we offer a flexible schedule, health insurance,

Qualifications

• Active State Licensure.
• Adherence to NBCOT/AOTA and State Required practice acts.
• Able to supervise and support physical therapist assistants; rehabilitation aides and/or students.

Responsibilities

• Screen/evaluate patients and develop appropriate plan of care following cognitive/ADL disorders.
• Provides rehabilitative, skilled and medically necessary treatment to patients and documents progress.
• Supervise and support PTAs, aides and/or students; coordinate with Director of Rehabilitation Services.
• Maintain compliant documentation, discharge summaries and participation in required meetings.
