Job summary

Evolve Therapy Services, LLC is seeking an Occupational Therapist for an in-house Skilled Nursing Facility partner in North Platte, NE. The role focuses on planning, organizing, and directing occupational therapy services to ensure high-quality resident care in compliance with standards.

The Occupational Therapist will screen, evaluate, and develop treatment plans, coordinate with the interdisciplinary team, and supervise COTAs while ensuring proper documentation and billing practices.

Qualifications

• Active Occupational Therapy license in practicing state(s).
• Degree in Occupational Therapy.
• Ongoing continuing education and professional development to maintain licensure and certification.

Responsibilities

• Conduct regular screening of residents to determine best intervention.
• Evaluate residents and develop treatment plans with goals.
• Interpret evaluation findings to residents, families, and team members.
• Establish resident therapy schedules with other team members.
• Perform therapy according to plan and document/bill services.
• Oversee COTA and ensure documentation and treatments are proper.
