Job summary

Renewal Rehab is seeking a licensed Occupational Therapist (OTR) to provide comprehensive OT assessments and individualized treatment plans. The role requires documentation accuracy, collaboration with medical staff, and adherence to orders and guidelines.

Qualifications

• Graduate of an accredited university with a B.S., MA, M.S., or Doctorate in Occupational Therapy recognized by AOTA.
• NBCOT designation as OTR (state dependent).
• Current state license/registration for Occupational Therapy.

Responsibilities

• Provide comprehensive occupational therapy evaluation based on MD orders.
• Document findings on the evaluation form timely and accurately.
• Develop a comprehensive treatment plan with goals, frequency, duration, and modalities.
• Obtain MD orders for evaluations, treatments, recertifications, and discharges.
• Deliver therapy using appropriate modalities and adjust as needed.
