Job summary

Advanced Health Care in Vermont is seeking a licensed Occupational Therapist to assess and treat residents in a skilled nursing facility. This PRN role involves planning and implementing rehab programs to promote independence in daily living activities.

You will collaborate with an interdisciplinary team to provide comprehensive care, document progress, and educate residents and families on interventions and home programs. Commitment to high-quality, resident-centered care is essential.

Qualifications

• Active state OT licensure or eligibility.
• Clinical experience in skilled nursing or similar setting preferred.
• Compassionate, patient-centered care with strong communication.

Responsibilities

• Assess residents' physical, cognitive, and psychosocial functions.
• Develop individualized OT treatment plans based on assessments and goals.
• Provide interventions to improve ADLs, therapeutic exercises, and adaptive equipment use.
• Document progress and discharge plans per facility policies.
• Collaborate with physicians, nurses, PTs, SLPs for coordinated care.
• Educate residents and families on techniques and home exercises.
• Participate in team meetings and care planning.
• Stay current with best practices and regulatory changes.
• Maintain licensure and professional ethics.
