Job summary

Barre Gardens Nursing and Rehab in Barre, VT is seeking an Occupational Therapist (OT) - PRN to deliver rehabilitation services within our skilled nursing facility.

You will evaluate residents, design individualized treatment plans, and provide OT services to maximize independence and quality of life. Collaboration with the rehab team and education for residents and families are essential parts of the role.

Qualifications

• Degree in Occupational Therapy from an accredited program.
• Active Occupational Therapist (OTR/L) license in the State of Vermont.
• Strong communication and interpersonal skills.
• Skilled nursing, rehabilitation, or long-term care experience preferred, but not required.

Responsibilities

• Evaluate residents to obtain data for treatment planning and set measurable goals.
• Implement OT treatment programs based on residents' needs and abilities.
• Collaborate with rehabilitation team to coordinate therapy programs.
• Instruct residents, families, and caregivers in OT techniques and plans.
• Monitor progress and modify treatment plans as appropriate.
• Maintain accurate documentation per facility and regulatory requirements.
